Medical Records Required for Clinic Appointments

If your child will be seeing the physicians at the convention, you will need to send in medical
records for your child(ren) by  May 31st, 2010.  

The medical records must consist of the following in order to get accurate answers from the top
physicians:  

* Ultrasounds, MRI’s, Cat Scans and any other testing completed, preferably on a CD/DVD (if
possible - usually the office where the test was completed will provide you with a copy of the
CD/DVD at no charge)  and the official reading or results from the physician who read it.  

* All Growth Curve Charts  

* Important Genetic Notes  

* Important Doctor Visit Notes  

Try to keep the notes condensed into 3 or 4 pages (which we all know is really hard to do), but
Dr. Dobyns has requested to try to pick the best, most important records and send 10 pages
maximum if possible.  

Please send the records separated into the four categories listed above and organize into
chronological order!  If your child is not attending, but you have an appointment with the
physicians please send in pictures and/or videos of your child.  Please also make sure to send
copies of the records and keep the originals for yourself - we are not able to return the medical
records.

records in Dr. Dobyns name -  Dr. Dobyns is not listed to receive mail at our mail box and could
