“We Are Not Alone” – the 4th Microcephaly, Lissencephaly, & PMG Convention is back in Arizona for 2012 and will
be held in Phoenix, at the prestigious Arizona Biltmore.

The convention will begin the evening of June 14th and the fun will continue all the way through the afternoon of
June 17th.  We welcome all children and their families who are affected by Microcephaly, Lissencephaly,
Polymicrogyria and other closely-related neurological disorders to join us for this special event.

If you would like an appointment with Dr. William Dobyns and the other top physicians attending, please check the
appropriate box on the registration form.  There is a $40.00 surcharge for a doctor’s appointment.  After we
receive your registration form and payment, we will contact you to set up an appointment time.  We will also
inform you of all the necessary medical records you will need to submit (at least 30 days prior) to your
appointment at the convention.

Convention and room reservations begin February 2, 2012 and are now available!  Register today and receive
“Early Bird” pricing!  Please keep in mind that transportation costs (airfare, rental car, meals not provided in our
plan) will be your personal responsibility.

Resort & Room Information

Closest Airport: Phoenix Sky Harbor International Airport (PHX) (http://www.phxskyharbor.com/)

Check-in and Check-out times:
Check-in time is 4:00pm.
Check-out time is 12:00 noon.

Pre and Post Rates:
The resort will be happy to extend the special rate for FCM guests: three days prior and three days after the program dates based on
availability.  So come early and stay after the
convention to enjoy all that Arizona has to offer.  Rooms need to be booked through FCM directly
to receive the special rate.

Food/Meals:
There will be five (5) meals included in your registration fee. All other meals will be your responsibility. Upon arrival, FCM will provide you with
a list of local restaurants at different price points and locations that your family may choose from, or you can go out and explore on your own
and find a hidden P
hoenix gem! Let’s not forget the option of having pizza delivered to your hotel room, or going down to the local grocery
store and picking up items to snack on in your room!

Cancellation Policy:
Full refunds for Registration Fees will be granted on or before May 1, 2012.  All requests for refunds MUST be made in writing, addressed to
